What you'll need
Have these ready before you sit down with the form. Incomplete forms slow down your application.
- A recent passport-size photo, attached to the top of the form
- Your national ID number, plus your driver's permit number and class if you have one
- Your NIS number and BIR number
- Your schools and qualifications, with dates (primary, secondary and any tertiary)
- Your last two employers: company, position, dates, salary and reason for leaving
- Three references with phone numbers, and an emergency contact
- Your signature and the date at the bottom of the form
